Please click on the attachment links below to obtain the membership form & minor release form. You must complete both forms below. Membership form and minors release form. Complete the form and mail to the OHSRA Secretary with the correct payment or bring it with you to the next rodeo! Don't forget you must enter the rodeo to participate! Entry forms for each rodeo are posted in the rodeo schedule. We invite you to join at anytime during the competing year.
You must also submit a current grade card, along with a copy of your birth certificate, to be considered a member. Please try to print forms front to back, as per request from NHSRA...thanks!
